Thanks for the replies. I am using a fairly basic 2 channel "arduino" style relay with optocoupler Hobbycomponents HCARDU0043 to be precise. I take 5V and ground from the Duet2 expansion port and then use the Fan2 Ground side?? connector to bring the relay "in" pin low and activate the relay.
If you are connecting all 3 channels of the relay board to the Duet in the same way then I can think of a few possibilities:
Fan 1 output mosfet blown short-circuit. You can check this by connecting a fan to the output instead of connecting the output to the relay. Bad connection between the relay board input of that channel and either FAN1- or +5V (or those wires swapped) Blown (i.e. open circuit input) optocoupler on the relay board, or bad soldering of the optocoupler or the input connector. Check this by swapping two relay channels.
