Failed upgrade

Does anyone know how to troubleshoot a firmware upgrade? I have a Duet Ethernet. It fails updating every time. It stops responding to web and serial after updating. If I turn it off and back on it reverts to the old firmware. It is currently on 1.19 with the newest web server.

Check that you have the up to date version of iap4e.bin in /sys on the SD card.
